UCLA Alzheimer’s and Dementia Care Program wins innovation award

The SCAN Foundation, devoted to transforming care for older adults, made its inaugural Innovation in Health Care Award to the UCLA Alzheimer’s and Dementia Care Program for its development of “a clear roadmap on how medical care and community-based service providers can partner to meet the needs of high-risk older adults and their caregivers.” The UCLA program, founded in 2012, partners with 8 such organizations to deliver coordinated, one-stop care. David Reuben, MD, chief of the UCLA Division of Geriatrics and director of the program, called the award “a tremendous honor.”
